By participating in any training or activities at Granite Bay Strength & Conditioning ("GBSC"), I ("Participant") acknowledge and agree to the following.
Eligibility
Participant affirms being at least 18 years of age and completing this Waiver on Participant's own behalf, for Participant's own participation only. This Waiver may not be completed on behalf of another person, including a minor. Participants under 18 may not use this form and must have a parent or legal guardian complete a separate waiver on their behalf prior to participation.
Health Warranty
Participant confirms being in good health and free from any condition that could make exercise unsafe or increase the risk of injury.
Assumption of Risk
Participant understands that fitness activities involve inherent risks, including the possibility of serious injury. Participant voluntarily accepts all risks associated with training at or through GBSC.
Waiver and Release of Liability
Participant releases and holds harmless GBSC, its owners, officers, employees, coaches, trainers, agents, and independent contractors (collectively, "Released Parties") from any claim, loss, injury, illness, or damage arising from the use of GBSC's services, equipment, or facilities, participation in any GBSC-related activity, or the use of techniques learned during training, including claims resulting in whole or in part from a Released Party's negligence. This release is binding on Participant's heirs, executors, administrators, and assigns.
Emergency Medical Authorization
In the event of an injury or medical emergency during participation, Participant authorizes GBSC staff to contact the emergency contact listed below and to contact emergency medical services, and, if Participant is unable to communicate, to share relevant information with emergency responders.
Severability
If any provision of this Waiver is found to be unenforceable, the remaining provisions will continue in full force and effect.
Electronic Signature
Participant consents to conduct this transaction electronically and agrees that Participant's electronic signature below is intended to authenticate this writing and has the same legal force and effect as a handwritten signature.
